چکیده

The haemodynamic effects of dopamine and isosorbide dinitrate alone and in combination were studied in 8 patients with chronic congestive cardiac failure. In 7 patients dopamine, 6,tg/kg per min, increased mean cardiac index from 2-0 to 3 0 1/min per m2 (P < 0.0005); mean stroke volume index from 20 to 27 ml/m2 (P < 0.0025), and mean left ventricular stroke work index from 14 to 20 g M/M2 (P < 0.0025). Mean left ventricular filling pressure did not change significantly. Isosorbide, 5 to 10 mg sublingually, reduced mean left ventricular filling pressure from 29 to 24 mmHg (P < 0.0005) and mean arterial pressure from 80 to 73 mmHg (P < 0 01), with no significant change in mean cardiac index, stroke volume index, or left ventricular stroke work index. When dopamine was reinfused after isosorbide administration mean cardiac index increased to 3 2 1/min per m2, stroke volume index to 31 mu/m2 (P < 0 05 vs dopamine alone), and left ventricular stroke work index to 23 g in/m2 (P < 0-0125 vs dopamine alone). Mean left ventricular filling pressure rose slightly to 25 mmHg but this was significantly less than for dopamine alone (P < 0 0005). In an eighth patient, whose left ventricular filling pressure fell to 12 mmHg after isosorbide, the reinfusion of dopamine was associated with severe bradycardia and hypotension, which responded to the intravenous administration of atropine. Afterload reduction with isosorbide combined with inotropic stimulation with dopamine may produce greater improvement in left ventricular performance in patients with congestive cardiac failure than either drug alone. Such treatment should be used with caution possibly in patients whose left ventricular filling pressure falls into the lower range after isosorbide alone.

Haemodynamic effects of oral salbutamol alone and in combination with sublingual isosorbide dinitrate in patients with severe congestive cardiac failure.

To examine possible augmentation of the effects of isosorbide dinitrate by salbutamol, haemodynamic measurements were made in 10 patients with severe chronic congestive cardiac failure who received isosorbide dinitrate 2.5 to 25 mg sublingually and salbutamol 4 to 12 mg orally, alone and in combination. Isosorbide dinitrate reduced mean left ventricular filling pressure from 29 to 18 mmHg and i...

Vasodilator treatment with isosorbide dinitrate and hydralazine in chronic heart failure.

Several reports have suggested that because isosorbide dinitrate and hydralazine have different and additive haemodynamic effects at rest in patients with chronic heart failure, these agents should be administered in combination. Some studies, however, indicate thay they are effective individually as well. Since most patients with heart failure are symptomatic only with activity, we examined th...

Comparative haemodynamic and peripheral vasodilator effects of oral and chewable isosorbide dinitrate in patients with refractory congestive cardiac failure.

venous capacitance, and calf blood flow were compared in a double-blind trial in 18 patients with refractory chronic congestive heart failure with pulmonary artery end-diastolic pressures >18 mmHg. Cardiac output and right heart pressures were measured with a Swan-Ganz thermodilution catheter, arterial pressure through an indwelling radial artery cannula, and venous capacitance and calf blood f...

Acute haemodynamic comparison of amrinone and pirbuterol in chronic heart failure. Additional effects of isosorbide dinitrate.

A randomised, within patient comparison was made in patients with severe chronic heart failure, to study the acute haemodynamic effects of oral agents which have inotropic and vasodilator properties. A non-glycosidic non-adrenergic positive inotropic agent with vasodilator properties (amrinone) was compared with a beta-agonist which has vasodilator and positive inotropic effects (pirbuterol). T...

Vasodilator therapy for chronic left ventricular failure.

The effects of chronic oral vasodilator therapy were studied in a group of patients with refractory congestive heart failure. Fifteen patients were treated acutely with intravenous sodium nitroprusside and sublingual isosorbide dinitrate. After continuous therapy with nitroprusside and isosorbide dinitrate for up to 72 hours the patients were then placed on isosorbide dinitrate and oral phenoxy...
